While it may be a bug within angular, it is not unusual to have save/exit silently fail. There are any number of reasons why this can happen from a holding template failing to having an empty input well for a call number or an item, to a duplicate barcode entered. Since the holdings editor closes, you have to start all over and hope you don't make the same mistake again.
