As for your point #1, I'd call that a "bug" rather than a style change. Our old/current docs approach relies on the root files to use "leveloffset" when including certain files to correct the heading level mismatch. Let's take this opportunity to fix that. Your suggestion of "upgrading" those headings sounds good; that's exactly what the "leveloffset" approach did.
