All the incidents I've investigated failed during search, never on copies.

I had an interesting one just now:

query: overdrive search_format(ebook) site(BC_ILC) pref_ou(BNA)
database query duration: 7989.742 ms

This is the first example I've had where the db query took >1s (not too surprising, this is a very broad search with a format limiter). This time, there are a few additional requests between "pref_ou = " and "flushing data from socket" messages, and there are four "flushing data from socket" messages instead of just one. That's slightly different from previous incidents, all of which have been a close match for my examples in comments 18-20 above.
