Observations:

- Crashes only seem to happen with multiple interfaces = "multiply defined PV" messages.
  Configuring the client to use only one NIF (localhost) and AUTO_ADDR=NO I cannot reproduce the issue.
  Setting AUTO_ADDR=YES I see segmentation faults and
    epicsThread: Unexpected C++ exception "epicsMutex::invalidMutex()" with type "N10epicsMutex12invalidMutexE" in thread "ipToAsciiProxy" at ...
  events.
