My original theory is wrong on both fronts.

First using leader-set on the non-leader node appears to have always exited with an error code, so this is not new.

Second our charms (at least rabbitmq and ceilometer) do gate based on leader. So we should never have leader-set executed if the node is not the juju elected leader.

This would start to point the finger at is-leader giving a false positive. But I am still collecting science.
