    upgrade: Ensure ceph-mgr is bootstrapped

    When upgrading to Luminous, ensure that the ceph-mgr daemons
    are bootstrapped *once* all ceph-mons have been upgraded.

    This ensures that the call to create the required keyring
    supports the new profile for the mgr daemon.
