Those buttons are never disabled even if you make no changes you can
still click them. Are you sure they are disabled?

Perhaps “disabled”” is an incorrect word. They are not greyed out or appear to not be available. It is simply that I am unable to click on them. I went back to Version: 6.11.0 and all works fine to edit TOC entries in edit book

I just downloaded, installed calibre version 6.13 macOS ventura13.1.

The same bug reported earlier still exists.

I am unable to edit TOC.

I have noticed that the issue with the button freeze to save changes happens when the dialog “select a destination for the Table of contents entry” is displayed. I can not enter a name of the TOC entry. I can not click on a different destination. I can not click on OK Or cancel. If I do escape or command - . then I can get out of the dialog. no changes made during the edit TOC were saved.

Note I can make changes on the first window to edit Table of content names or reorder. If i make changes here and click OK those changes are saved. Only if I attempt to create a new entry do I get issues in the dialog

attached is a movie file.
