Bug Description
Since Calibre 4.1, setting the cover from PDFs quite frequently fails, since some of the pages shown in the selection dialog are shown as blank and inserted as blank pages. In fact they are not blank. In the attached example PDF the selection dialog shows 10 blank pages. This worked in a prior version.
Calibre 4.1 with Ubuntu 19.04
