Bug Description
Calibre version 4.1 Portable version fails to generate cover from PDF file first page for most of PDFs.
It was working properly with version 3.*

Having the same issue. Calibre 4.1 on Moc OS X 10.14.6.
Most PDFs show a solid white JPG as the cover. If I go to set the cover of the book from the format (PDF) all the pages it offers are just white JPEGs. The PDF itself was fine.
As Tamsusis reports, it always worked fine on versions 3.x of Calibre. Seems to have broken with the release of 4.0 and is not fixed yet in version 4.1.
