Unable to add new color scheme

Bug Description

Whenever I click on the new scheme button in 'Colors' section it gives me the following error:

Uncaught TypeError: Cannot read property 'focus' of null
Error at userscript:viewer.js:24264:55
Traceback (most recent call last):

Calibre version:4.1 64-bit
