Please include with all bug reports, where they're relevant:

- Your calibre version
- If you aren't on the latest calibre, can you replicate this with the latest calibre version?
- If you aren't able to update to the latest calibre version, why not?
- The plugin name and version (found on the list of installed plugins) (because there's two plugins covered by this project)
- A sample book, both the source ePub and the generated KePub, demonstrating the problem

- Are you using the Document Default font, another included font, or a side-loaded font?
- If your problem is related to layout, and you're using a custom CSS file, attach your custom CSS file
