No sound output except for headphone output

Description:
In 11.10, my audio worked perfectly.
However, in 12.04, on my intel HD audio, I can only get output from the front panel headphones output. None of the outputs on the back panel work.
There are some unusual symptoms. For example, the "headphone" checkbox is always checked, and if I uncheck it, it mutes all sound (systray icon shows muted). Even if I mess with all sliders, muting and unmuting them, the back outputs won't work.
Attempts to fix, in no order:
-run from liveCD
-purge + reinstall alsa and pulseaudio
-all combinations of settings in alsa-mixer, pavucontrol, sound control panel
-disable + re-enable intel-hda in BIOS
-all suggestions on the soundtroublesho

I forgot to mention that under sound settings modes: there is no "headphones" settings, but there are all the other speaker output modes (duplex 4.0, 5.1, etc), and selecting any of these modes does not help.
