I had removed the audigy 2 zs notebook pcmcia soundcard while the volume control program was open. Switching between my laptop's inbuilt and the zs caused a crash (the zs didn't disappear as a device, just showed an empty volume control, then crashed).

Yep, I can't troubleshoot that anymore, as I don't have the hardware.
