./setup.py sphinx_build reports 43 issues

Several weeks ago, this output was clean, and has since exploded into a mess of ERROR's, WARNING's, and SEVERE issues.

    $ python setup.py build_sphinx

This is resulting in broken links, missing documents and poor formatting on http://keystone.openstack.org/ as the output above indicates.
