a radio button on this PDF form makes evince exit with an error

Bug Description

Binary package hint: evince

Steps to reproduce:
1. evince Computer-LaptopRequestForm-edit_v2.pdf
2. click the Staff radio button, and BLAMMO:
Error (0): Call to Object where the object was type 4, not the expected type 6
Aborted

This is the worst sort of bug, to my mind: it throws away the information I just
entered into the form.

Thank you for taking the time to report this bug and helping to make Ubuntu better. This particular bug has already been reported and is a duplicate of bug 700852, so it is being marked as such. Note that bug 700852 has been marked as "Fix Released" because the issue is fixed in Ubuntu 11.04. If you need a fix for the bug in previous versions of Ubuntu, please do steps 1 and 2 of the SRU Procedure [1] to bring the need to a developer's attention.

Any further discussion regarding the bug should occur in the other report. Please continue to report any other bugs you may find.
