libcdk5-dev missing header files used in examples

Binary package hint: libcdk5-dev

Examples provided in package libcdk5-dev in directory /usr/share/doc/libcdk5-dev/examples include the file cdk_test.h which is missing from both libcdk5 and libcdk5-dev.

I'm including a patch for the package source. I've patched it against the current apt-get source package and it now includes the missing header file in the examples directory, as well as patched Makefile in the source directory.
