All audio stops when playing audio/video in firefox, killing FF restores audio

Bug Description

Binary package hint: firefox-3.5

Apologies if this isn't the correct package to report on.

Audio works great on my system until I try to play audio or video in Firefox.
I am able to use vlc, mplayer, audacious, etc...to play a wide variety of audio file and audio as part of a video without issues.

If I start a new firefox session, things continue to work correctly.

In that new session, if I surf to youtube (for example) and attempt to watch a video, I get no sound output a few frames of jerky video and then the video hangs.

The following events go into my syslog:
Oct 9 23:20:53 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.
Oct 9 23:20:53 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.
Oct 9 23:20:53 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.
Oct 9 23:20:53 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.
Oct 9 23:20:53 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.

If I switch away from firefox to mplayer, vlc, etc...I am able to play movies, but I get no sound.

If I go back to firefox and quit, the firefox interface disappears but the firefox process is left running. At one point I left it for 15 minutes and finally had to kill -9 it.

Right after I kill -9 firefox, I get the following three events in my syslog:

Oct 9 23:21:07 hoth pulseaudio[21455]: ratelimit.c: 2 events suppressed
Oct 9 23:21:07 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.
Oct 9 23:21:07 hoth pulseaudio[21455]: sink-input.c: Failed to create sink input: too many inputs per sink.

And I am suddenly able to play sound in all my apps again.
