pulsaudio version: 0.9.14-0ubuntu4: 10.0.15.3ubuntu3
firefox version: 3.0.6+nobinonly-0ubuntu1

Firefox crashes on all attempts to use Flash Player, including YouTube and most Flash Games (www.kongregate.com).
The page is generally displayed for about 5 seconds on average, occasionally with a little of the audio beginning. The audio in this case is very choppy, with a lot of background noise. The audio will not play for more than half a second before Firefox crashes.

Firefox crashes with terminal output:
ALSA lib pcm_pulse.c:626:(pulse_prepare) PulseAudio: Unable to create stream: Invalid argument (repeated about 10 times)

E: shm.c: mmap() failed: Cannot allocate memory (repeated about 30 times)

firefox: pulse.c:200: pulse_new: Assertion `p->context' failed.
Aborted (core dumped)

All the text appears in the half-second when the audio occasionally plays. It appears even when the audio does not start.

Hello,
I had this problem and noticed I had flash version 9 installed.
On upgrading flash to version 10 embedded video playback works again.
