add fields: badge line 1 and 2

badge line 1
badge line 2

Badges have 2 lines for the attendee to put whatever they want.

can be used for:
City where they are from
project they are working on (url)
personal website/blog
email address
favorite python module
favorite monty python quote

We need the fields used on the badge labeled so that people understand that is
what they are for.

If we need to collect company name, then we should have a field for that too.

If the person wants to put the name of their company in all 3 fields, that's
fine. But they should have the option of putting different values.

The web form currently has a "Nickname" field. This should be "Name on badge". It can continue to be auto-filled from given & family name.

The "Badge line 1" and "Badge line 2" fields are also necessary. We cannot assume that any particular piece of info should be on a badge. In other words, they should be independent fields. Some people will want no more than their name. Others will put their email address and/or company and/or project etc. Some will NOT want one or more of these bits of info on their badge. We should allow this flexibility.

Please group these 3 fields together, and order them as on the badge: name, line 1, line 2.
