Survey questions on Patron Edit screen lack labels

Bug Description
As Keith Huan noted in the comments on bug 1615714, the survey questions at the end of the Patron Edit screen do not have labels associated with their inputs. This prevents screen reader users from being able to fill out the fields.

On further inspection, these aren't even form fields at all, but Bootstrap dropdowns with click actions attached to the list items. These should be <select>/<option> elements instead. The relevant code is around line 1209 in Open-ILS/
The patron groups have the same issue (line 594 or so of the same file).
