web 3.1.10 creating duplicate holds that aren't wanted

Bug Description
We are using the web client 3.1.10 on Chrome. Staff report patrons receiving multiple copies of items to fill their holds. I saw this behavior twice last week in my own account and in the account of a patron I was helping. From the staff side, I placed one hold, not changing the number of requested items from the default of 1, and then saw in the patron records that two items were on hold.

I'm inclined to mark this a duplicate of bug 1029601. Before I do, could you confirm that this is what is happening, Diane? Are you double clicking on the submit button?
