[nouveau] icons disappered, system unusable

Bug Description

the launcher bar goes into miniscule size most icons are gone (all icons like close button etc. look like "broken image" icon, all icons near the menu, where you can log out including the bar) login does not work and commands do not run or for example sudo throws error (I had to hard restart pc), but I do not remember the message. Cannot find anything, that would trigger it (I had some system programs failing before this happened). I cannot provide screenshots, becouse I do not know how to take one on my Zbook while not using external keyboard.

Cannot provide screanshot. Fn rshift /super fn rshift does not work and
Zbook does not have dedicated prt scr button. 2. Loggging out/in did not
help. Cannot test the suspend/wake up, becouse i had to force restart pc to
file the bug. I will be able, when the bug occures again.

Instead of most icons in app menu there are three dots. Few are intact.
Side panel has nearly circles instead of icons with circular corners. Size
is about 1/5 of normal size. Will try to diagnose further when it will
happen again.

Yes those are two separate issues. Let's make this bug about the missing buttons.

The problem with the terminal window title bar looks related to bug 1718238.

Maybe it might be bed connection between flash disc and usb port becouse I have seen something simillar after gnome disappered.

If you have any nonstandard gnome-shell extensions installed then please start by uninstalling them.

Note also that you are using an Nvidia P1000 GPU (very new) with the nouveau driver. To the best of my knowledge that doesn't work at all for other people (bug 1822026).

You might get a better result using the proprietary Nvidia driver:

  sudo apt install nvidia-driver-418

i thing that this bug is somehow triggered the same way as the bug with non running commands. If any bug of this family happends I will post the log. else I will post a log which is from 4.4.2019 to today, where the bug is burried deeply in some days before today. The latest one to two errors is me trying to replicate wrong connection between disc and pc by partially removing the disc connector which I thing reproduced the issue qite welll. now installing propper display drivers.

It started happening while I was avay from the computer. Backspace was stuck on and delete every character of password and after it crashed wit read only file syster error spamming something in console I did not take picture. Might be different error/bug.

I was avay from computer when i trued to log in it was deleting my password so couldnt login and it was spamming incorrect login and after some time it crashed. Hope, it is not my usb storage dying.

Yes the screenshot in comment #19 shows you have a disk (sdb) that is corrupt/faulty/dying. You will need to remove or replace that.

I an now tester errors on first two partations. No errots. Will try the
last, but that is mounted to / so must use my usb disc with installer to
test it. If errors I will do backuf, reformat and reinstell. Else will need
new flasj disc.

Ubuntu 19.04 (disco) reached end-of-life on January 23, 2020.
