At a glance, a couple of things. There is no real need to mention that you have re-added the ubuntu changelog entries in the changelog. You also have not mentioned that the maintainer field has been updated as per the debian maintainer field specification. It may be mentioned in earlier entries, but since it is an Ubuntu specific change, it still needs to be mentioned.

Please re-subscribe ubuntu-universe-sponsors when you have attached a new debdiff. If you could also provide an URL for the orig tarball, that would also be appreciated.
