Bug Description
Tested in Firefox, Chrome and Safari5, via php5
Test steps:
Collection>SE matrix is displayed
Click on icon to open the Annotation
click on insert/edit image
select image from Group file>image is selected
click again the Image button to Remove the attachment
X[[[remove]]] is displayed
