Bug Description

I install Linux onto USB mem-sticks, but this is the first time Ubuntu has failed like this. These are 16Gb ADATA sticks and have never failed me before. One is unusable now and I haven't been able to check the second one yet.

This appears to be some sort of hardware error. If it isn't the usb sticks, then it could be your hub or controller. According to your logs, the drive simply hung up for a few minutes, and the system tried to reset it, but it still failed to respond, and so was declared dead.

I have used these sticks to install Linux before, so something in the
Ubuntu installer is likely to have triggered something unusual.
