Create a project HTML5 dialog | incorrect title and double listing

When creating an HTML5 App project in the Ubuntu SDK, it sometimes lists www/index.html multiple times and has a window title of "New Ubuntu QML Project", which is incorrect as it is an HTML5 App project. (See attachment)

Yes, I don't like that either, but this is due to our re-using of the ubuntu plugin code. The solution is to create a different project type in the plugin code.
