KEPUB output plugin fails to convert ePub to KePub

Bug Description

Hi,

This new bug is a follow-up of bug 1252050.

After all, the plugin seems to be installed even if it doesn't appear in the extensions window's "Installed" tab.
I can say that because the extension appears in Calibre's preferences window (section "Extensions->Conversion extensions (output)").

However, then, if I try to convert an ePub book into KePub, then the conversion fails.
Conversion log is attached.

I'm using Calibre 1.12 (upgraded to every release since 1.07) and just installed KePub Output plugin 2.2.13 (after removing all the installed plugins and checked that ~/Library/Preferences/calibre/plugins was empty).

Plugin version 2.2.12 was working perfectly in Calibre versions up to 1.10 and started failing in Calibre 1.11

The attached log has been generated in Debug Mode Calibre.

This is a calibre bug, fixed in the next calibre release.
