LBaaS "Add Health Monitor" and "Delete Health Monitor" links are not displayed as pop-ups

LBaaS "Add Health Monitor" and "Delete Health Monitor" links don't open pop-up windows now. That's why there are no "Cancel" buttons there. It would be nice to solve this problem by adding "ajax-modal" class to these links.

    Fix "Add Association" and "Delete Association" displaying

    This patch adds "ajax-modal" class to LBaaS "Add Health Monitor"
    and "Delete Health Monitor" links. So these links open pop-up
    windows with "Cancel" and "Add"/"Delete" buttons.

    Fixes bug 1212978
