UI changes for Mahara 1.8

Mahara 1.8 gets a new set of monochrome icons. This is the primary bug number for these changes. Some other changes will be on other bug numbers such as flexible page layout and artefact chooser.

Problem when the block instance has too longer string it overlaps things to it's right.

One of the UI commits made it impossible to add pages to a new collection. The problem was that while rearranging the elements in the collection/views.tpl page template, Evonne moved the Page collection form inside of an {if} block that it was previously outside of. So, I've pushed a fix patch:

Also, other than that regression, I like the now pages-in-collections form's look! :)

If other "picker" pages went through a similar re-styling, we should make sure they don't have a similar bug.
