Bug Description
Fields shown in Add Monitor form should depend on type of monitor. For PING and TCP types fields 'HTTP Method', 'URL', 'Expected Codes' are not needed and should be hidden. Moreover, if 'Expected Codes' is clear then the application crashes with error: "Invalid input for expected_codes. Reason: '' is not a valid input."
