nvidia card falls off bus when playing video, causes Xorg crash

Bug Description

This apparently has caused others grief in the past as far back as nvidia-295, but now I'm getting it, and the only way to get the system back is a cold reboot. The X server crashes leaving me with no video, and even ssh-ing in from a remote machine I cannot reboot the system (running reboot does nothing). Whenever it happens, I see the following in syslog:

Dec 25 06:42:25 echo kernel: [ 3348.820000] NVRM: GPU at 0000:01:00.0 has fallen off the bus.

This happens within 5 minutes of watching any video. I have not been able to reproduce it without playing video. My system has been running fine with 12.10 until about a week ago.

I've also tried enabling persistent mode to no avail with /usr/bin/nvidia-smi -pm 1

FYI, this does not happen using the Nouveau driver, although it is unusably slow.

I've tested further and at this point I'm fairly certain this is a hardware issue - another nvidia card works perfectly in this machine. It's likely the card possibly overheated and died, causing all these problems.
