This happened following https://bugs.launchpad.net/ubuntu/+source/update-manager/+bug/1087502 when I tried to run do-release-upgrade a second time. Essentially duplicate, submitted in case auto-generated additional information is useful. This may be related to https://bugs.launchpad.net/ubuntu/+source/initramfs-tools/+bug/1035647 and I will try the workaround suggested therein.
