powervm: instance fails to spawn yet nova reports active

When spawning an instance on PowerVM, if the instance fails to be created on the compute host, nova is not informed of the error and reports the instance as Active. Expected behavior would be to try a reschedule and/or report the instance as Error status.

    powervm: failed spawn should raise exception

    fixes bug #1065295

    On a failed spawn instance, we should run our cleanup on
    the host and then re-raise the exception to inform
    ComputeManager of a failure
