Bug Description

If scheduling fails in anything other than run-instance, it doesn't properly create an instance fault or set the task state back to None.

    Improve error handling of scheduler

    Modifies scheduler errors to report instance faults and to set
    instance_state back to None on failure.

    Related to bug 1051066
    Fixes bug 1052993
