handle missing policy file gracefully

Bug Description

Currently, Glance won't start if it can't find its precious policy.json file. We should allow glance to start with a default set of policies and dump a big warning in the log.

    WARN and use defaults when no policy file is found

    Rather than refuse to start, it's nicer to use a sane set of default
    policies when no policy file is found.
