Bug Description

This is possibly a duplicate of bug #696693. But that doesn't mention multiple entries for the same user, and it doesn't include a screenshot, so I can't know.

On my system, this was happening because my user was a member of both the "admin" and "sudo" groups. The "admin" group is no longer used by Ubuntu by default (it is "adm" instead), so removing your user from the "admin" group should solve this issue. Your user should still be a member of the "adm" and "sudo" groups, though.
