Just to add another wrinkle: alerts on Files seem to be working. Alerts aren't going through for Blogs (unless content is emailed in), Wikis or Calendar Events.

It doesn't have to do with what kind of alert it is. It has to do
with which application server is handling the request that generates
the alert. Currently we have a misconfig where the new app servers
are unable to send mail. For any given alert you have a 1 in 3 chance
of it going out. Since all mailin is processed on the main app
server, alerts generated by mailin always go out, as you have
observed.

I'm composing a note to Gocept about the mail config. In the meantime
I might just jury rig something to periodically copy outgoing mail to
the main server so that it can get sent out.

Gocept has configured the MTA on osfkarl10 to accept mail from osfkarl11 and osfkarl12. I have tested and it looks like it's working. I've removed the band aid and we're back to using the regular mailout process on all three app servers.
