Nope, still persists. When opening a document directly from Nautilus, without splash screen (Logo=0), I get one of these scenarios randomly
1) Correct icon (empty document icon flashes but is replaced).
2) Correct icon + an empty space with no icon. Both disappears when the document is closed.
3) Wrong icon (empty document) but present in alt+tab switcher and window management works correct.
With Logo=1, I get 1-3, but also:
4) No icon and not present in alt+tab switcher.
*) Tested with LibreOffice Calc, and Unity 5.16.0-0ubuntu1 from precise-proposed.
