If the icons still are unavailable, please post the debugging output again. I suspect that the icons should work now though :-) The icon problem is then in the deb in that it doesn't execute gtk-update-icon-cache on install.
Regarding the notification issue, I have to investigate further. What's difficult is that it's so hard to reproduce. Approx how often does it display, and how often does it not display for you?
Thank you for your logs, did you install alarm-clock from source or deb?
If you installed from deb, could you please try to execute the following and then restart the applet (from terminal, as before):
$ sudo gtk-update- icon-cache -f -t /usr/share/ icons/hicolor/
