Fix build on Archlinux (gcc 4.6.1)

Bug Description

I've update AUR package to zorba 2.0.0 (http://aur.archlinux.org/packages.php?ID=20593).
I had troubles with compiling source (gcc 4.6.1), since in several headers size_t and/or ptrdiff_t were not defined. To fix it I just included cstddef.
Also I commented out doc/xquery from install, the installer failes and I don't see this folder.

The file fix_cstddef_include.diff was added: adds <cstddef> were required

The situation has changed since 2.0.0.
However, zorba 2.8.0 still doesn't compile out-of-box for me.
I have to #include string and stream headers into item_handle.h to make it work (see the recent PKGBUILD file on the AUR package page).
