Zim 0.44 : Exporting to HTML - Index page generation and linking is broken

Bug Description

When exporting to HTML, the index page functionality is broken.

1) Index.html contains no references to any pages.
2) All the links from wiki pages to the [Index] link are links to '/', of the form:

 [ <span class='insen'>Prev</span> ]

 [ <a href='/'>Index</a> ]

 [ <span class='insen'>Next</span> ]
----

The attached file is my test case - a small wiki with three interlinked pages. The zim pages folder and the zim-exported HTML version are included.

Fixed in rev219. Added test to double check index is actually generated - and linked it in the default template.

I'm using Zim 0.65 in Windows 10 and have a very similar problem exporting to HTML with:

1. Default_with_index.html template
2. Export each page to a separate file
3. An index page is specified

The problem is that all index page links are generated as:

<a href='index'>Index</a>

I attach a test notebook and generated html.
