Chapters registration page missing

We want to replace this anyway (see Bug 125185), but until it's replaced, the old one is AWOL. Unless we want to freeze or do everything by hand, we should probably restore the old one.

We actually moved that page to a different (secret) URL at some point, because the reg form was getting spam. We now will not reveal the reg form's URL publicly, so that we can prevent spam and ensure that people talk to us before trying to register a chapter.

However, that new URL is borked/missing as well. Attempting to visit that URL just displays our blog. Sad. Guess someone should go grab the file(s) from the old directory.

Technically, this bug is fixed, but I'd like to leave it open until we get the UI a little nicer. Triaged for now
