Please could you add more information to the bug. Please have a read of https://docs.openstack.org/charm-guide/latest/community/software-bug.html and submit at least: ubuntu version, vault track, juju status, and preferable crash dump which includes juju show-unit. Thanks.

After staring at this some more, this is essentially the same issue as https://bugs.launchpad.net/vault-charm/+bug/2007587, which is also about upgrading the vault charm.

Essentially, unless units are paused, then they fail when the upgrade-charm hooks fires and the underlying vault unit needs to be restarted. However, it also looks a bit like there's a problem with the certificates on the unit after the upgrade, so this looks like a general upgrade bug.

You mention in the original remote that you are attempting to "revert vault by upgrading rev 100 to 108" which doesn't sound like a revert, but an upgrade?

Upgrading the vault charm seems to be broken at the moment, so I'm triaging this as a high.

We(me, Alex) had a discussion about this that this is exactly a regression reverted.

usually leader is failed when upgrading, and removing unit and re-adding it can be workaround for this issue.

but I think it is quite burden for production.
