upgrade from 10.04 to 10.10 hangs with option screen behind window and can't change focus to that window

Bug Description

When trying to upgrade from 10.04 to 10.10 using update managers, the process hung when installing the packages. There was a second window open behind the progress window, but I couldn't select that window to change the options presented in in. The only thing I was able to do after waiting for about an hour was ssh in and I then ran

sudo dpkg --configure -a

to finishing configuring the package upgrades that hadn't completed.
This partially completed, but exited reporting errors. These errors were
being reported as /var/cache/debconf/lock was not released when the
update manager process was killed. Searching on launchpad I found the
simplest way to release the lock was to

sudo rm -rf /var/cache/debconf/*

Rerunning

sudo dpkg --configure -a

then completed without error. I was presented with a number of
configuration screens giving me the option to keep or replace a number
of configuration files (php, samba, rsnapshot, postfix etc)

Once this was completed I then reran apt-get to update, to ensure that
everything was updated, which also completed without error.

I was then able to successfully reboot into 10.10

I encountered this issue while upgrading 10.04.1 to 10.10 desktop as well. The installation halted with a prompt for "Configuring grub-pc" asking whether to proceed with or without installing grub. However, this window opened behind the Distribution Upgrade window, and at this point in the upgrade the windows had lost their title bars and borders, making them unable to be moved. Clicking the edge of the prompt window would not bring focus, nor would Alt-Tab. The installation was unable to proceed due to not being able to make selections in the "Configuring grub-pc" window.

The solution was to revert the machine to a previous state and re-attempt the upgrade, this time preemptively moving the Distribution Upgrade window to the edge of the display.
