Black box painted when logging into to Unity

Bug Description

When logging into Unity, the full environment appears as it should, but a black box is drawn in the middle of the screen. Usually when I log in I am asked for my default keyring password, and it looks like that instead of painting that window, it is just showing the black box in it's place. Unfortunately, when I click anything else it is unresosponsive (e.g. I click the indicators, and I can see the click happens (the indicator is selected), but no indicator menu drops down).

Interestingly, if I kill lightdm, log into Unity 2D, log out, and then back into Unity 3D, I see the normal keyring password box as it should display, and Unity works fine.

The black box I see is for the keyring password box. It seems the box just doesn't render correctly and it blocks the entire environment.

I have since discovered that if I go to the launcher icon for the password dialog, right-click and quit it, it then reappears and I can authenticate and then Unity works like normal.
