Dash recent list is not actually with recent programs and files

Bug Description

I am not sure if it is a but or just something I do not understand. In my understanding, when I press Ubuntu/Win/Dash button, the list of programs and files listed there should be either the most frequently used or the last ones used programs / files. I have just restarted the computer, so this is a new session and the list in dash is anything else but like that.

In the attached screenshot you see the first file/folder "RO.A.D.2013.40". I have no idea when I last opened that folder, but I think it has passed a few months. The programs list is not better. If I were to get the most frequently used programs, I would have got there Firefox, Chromium, Filezilla, Nautilus, Terminal, Ubuntu Software Updater, KSysGuard. Pretty much the same list for the last used programs. If it helps, many of those programs have shortcuts on the launcher bar.

Why do I get those in Dash? I cannot remember the last time I used Security&Privacy. Last time I used Inkscape could be one week ago, maybe more.

I am adding another screenshot where you can see unity launcher bar, it might help. Shutter is also a very frequent used program, but it starts automatically with Ubuntu, so I almost never start it by hand. Virtualbox is also quite often used.
