unity double application menus

Bug Description

this is random behavior
sometimes after reboot there are double menus for 1 application, global menu & same menu in application window. All menus & context menus (right click) gain icons.
Sometimes after reboot everyathing is normal.

changing theme = doesn`t help
changing theme or gui settings like show icons in menus = off using MyUnity, UbuntuTweak or Unsettings = doesn`t help
purging those utilities from the system together with their config files = doesn`t help
deleting .gtkrc & .gtkrc-2.0 = doesn`t help
unity --replace = doesn`t help
logout & login = doesn`t help
2-3 reboots = doesn`t help

please, if you need any other info just write & I am gonna send it

p.s.
running unity from terminal there are some vissible errors like:
$ unity --replace

run 'gnome-settings-daemon' from terminal and see if the menus still have icons or not. though the dual menus things is totally new for me.

Omer you where right. There is something wrong with:
gnome-settings-daemon

running it in terminal produced this:

i have these type of problem frequently but after updating and upgrading this bug vanishes
