Wrong title and buttons on unsuscribe dialog

Bug Description
the window title is incorrect and should be fixed.
Furthermore, we shouldn't be using this combination of button choices here. Yes/No/Cancel don't make any sense, and Yes/No should generally never be used.

We need to determine which text should be in the buttons and if we are going to maintain the current option with Yes/No/Cancel, or which should be the proper solution.
