OpenSC formatted Starcos 2.3 smartcard cannot be initialized

Bug Description

* What happened
Erasing and initializing a Starcos 2.3 smartcard returns the error:
Error -1211: Security status not satisfied

* Test case
$ pkcs15-init -E
$ pkcs15-init --create-pkcs15 --profile pkcs15+onepin --use-default-transport-key --pin 0000 --puk 111111 --label "Test"

* It is expected to properly format the card, and this can be verified by the command returning 0, and pkcs15-tool --dump properly displaying information about the newly formatted smartcard.

This works in 10.04LTS (opensc-0.11.12-ubuntu3.3).

In addition I've bisected the issue with the OpenSC git project and found that the first bad commit is:
1d82e8ababab85480aeffb955cad27267a938883

This issue exists with the latest version of OpenSC and thus is an upstream bug.

Ubuntu 12.04 (precise) reached end-of-life on April 28, 2017.
