Pulseaudio volume master breaks after plugging in headphones

Bug Description

The only way I've found to restore it is by rebooting. I can control the volume with amixer, and pavucontrol's per-application sliders work fine, but the master in pavucontrol doesn't work. (So naturally the volume keys don't work either.)
